Park View Rehabilitation Center in Sac City, Iowa is seeking a MDS Coordinator to join our friendly, caring and supportive team. You will develop, monitor, audit, and modify each resident's care plan to meet individual needs and goals, conducting assessments and assisting in the discharge process.

Qualifications

• Unencumbered Iowa RN license or LPN certificate.
• Graduate of an accredited nursing program.
• CPR certification.
• Prior MDS Coordinator experience preferred.
• One year RN experience in a Skilled Nursing Facility and one year CNA experience preferred.

Responsabilités

• Develop, monitor, audit, and modify each resident's care plan to meet individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Provide knowledge and guidance to the team to ensure high standards of care.
