Job summary

Park View Rehabilitation Center in Sac City, Iowa is seeking a MDS Coordinator to join our skilled nursing team. You will develop, monitor, audit, and modify each resident’s care plan while ensuring accurate assessments and a smooth discharge process.

Qualified candidates will hold an Iowa RN license or LPN certificate, CPR certification, and have completed an accredited nursing program. Prior MDS coordination and SNF RN experience are preferred; we are willing to invest in the right candidate.

Qualifications

• As a minimum, an unencumbered State of Iowa RN License or LPN certificate, CPR Certification, and graduation from an accredited nursing program.
• Prior experience as a MDS Coordinator and RN experience in a Skilled Nursing Facility is preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Provide knowledge and support to the care team to maintain high standards of care.

As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Park View Rehabilitation Center by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License or LPN certificate, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
