Park View Rehabilitation Center in Sac City, Iowa, is seeking a full-time MDS Coordinator to join our caring team. You will develop and monitor care plans, perform resident assessments, and assist in discharge planning to ensure high-quality resident care.

Ideal candidates hold an Iowa RN license or LPN certificate, have graduated from an accredited nursing program, and possess CPR certification. Prior MDS experience and SNF RN experience are preferred; we invest in capable professionals.

Competenze

• Prior experience as a MDS Coordinator.
• RN experience in a Skilled Nursing Facility setting is preferred.
• CNA experience is valuable for understanding resident care needs.

Mansioni

• Develop, monitor, audit, and modify each resident’s care plan.
• Conduct resident assessments and assist in the discharge process.
• Ensure residents receive high standard of care and safety through accurate documentation and coordination.

As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Park View Rehabilitation Center by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License or LPN certificate, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
