Job summary

Nemasket Rehabilitation and Healthcare Center in Massachusetts seeks an experienced MDS Coordinator (OSHA Classification 1) to ensure complete, accurate, and timely clinical assessments in collaboration with the interdisciplinary health care team. This role supports short and long-term improvement goals and directs care planning.

The position requires an LPN/RN license, at least 3 years in geriatrics, CPR certification, and strong MS Office skills.

Qualifications

Must be a licensed LPN/RN in CT; 3+ years geriatrics preferred.
CPR certified; able to read, write, and follow instructions.
Proficient with MS Office and office equipment.
Able to work hours as assigned.

Responsibilities

Maintains care conferences calendar and conducts interdisciplinary meetings.
Performs comprehensive resident assessments via records, interviews, and evaluations.
Completes MDS with RAPS and resident summary; develops care plans.
Ensures timely RAI processing and regulatory transmission.
As applicable, lead case management for Managed Care.
Participates in CMI and billing meetings; communicates PDPM recommendations.
Reviews Medicare resident for appropriate coverage.
Maintains monthly Quality Indicator Reports and confidentiality.
Attends meetings as assigned by DNS/ADNS.
