Job summary

Eastcastle Place is seeking an experienced MDS Coordinator to own the MDS lifecycle from assessments through reimbursement. You will work under flexible hours with a 28–40 hour work week, coordinating with nursing leadership, therapy, social services, and other disciplines to ensure timely, accurate documentation and survey readiness.

The role emphasizes ownership of the MDS process, meeting deadlines, and maintaining high data quality, while collaborating across departments to optimize resident

Qualifications

• MDS experience: minimum 2 years in skilled nursing or long-term care.
• Current Wisconsin RN license and ability to work in a regulated environment.
• Strong attention to detail and organizational skills for regulatory readiness.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments.
• Oversee the MDS assessment and care-planning process.
• Manage assessment schedules, documentation requirements, and submission deadlines.
• Review clinical documentation for completeness, accuracy, and consistency.
• Identify documentation gaps and follow up with appropriate team members.
• Collaborate with nursing, therapy, social services, and other interdisciplinary team members.
• Develop and update comprehensive, resident-centered care plans.
• Monitor documentation to support regulatory compliance and appropriate reimbursement.
• Provide education and guidance to nursing staff regarding documentation standards and MDS requirements.
• Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
• Support survey preparation and ongoing survey readiness.
• Participate in quality improvement and clinical initiatives.
• Stay current with applicable federal and Wisconsin MDS requirements and regulatory changes.
