Job summary

Eastcastle Place is seeking an experienced MDS Coordinator to drive the MDS process, including assessments, documentation review, and interdisciplinary care planning. This role emphasizes ownership, deadline management, and collaboration with nursing leadership and other departments.

The position offers a flexible schedule within 28–40 hours per week, with a strong focus on accuracy, survey readiness, and regulatory compliance to support resident care and reimbursement.

Qualifications

• Active RN license in Wisconsin requirement.
• 2+ years MDS experience in skilled nursing or long-term care preferred.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments.
• Oversee the MDS assessment and care-planning process.
• Manage assessment schedules, documentation requirements, and submission deadlines.
• Review clinical documentation for completeness, accuracy, and consistency.
• Identify documentation gaps and follow up with appropriate team members.
• Collaborate with nursing, therapy, social services, and other interdisciplinary team members.
• Develop and update comprehensive, resident-centered care plans.
• Monitor documentation to support regulatory compliance and appropriate reimbursement.
• Provide education and guidance to nursing staff regarding documentation standards and MDS requirements.
• Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
• Support survey preparation and ongoing survey readiness.
• Participate in quality improvement and clinical initiatives.
• Stay current with applicable federal and Wisconsin MDS requirements and regulatory changes.
