Eastcastle Place is seeking an experienced MDS Coordinator to own the MDS process in our skilled nursing community. You’ll manage assessments, documentation review, interdisciplinary coordination, and reimbursement while ensuring survey readiness.

Qualifications

• Current, unrestricted RN license in Wisconsin.
• Minimum of 2 years of MDS experience in skilled nursing or long-term care.
• Strong organizational and documentation skills.
• Ability to manage multiple deadlines and work with interdisciplinary teams.
• Experience preparing for state surveys preferred.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments.
• Oversee the MDS assessment and care-planning process.
• Manage assessment schedules, documentation requirements, and submission deadlines.
• Review clinical documentation for completeness and accuracy.
• Identify documentation gaps and follow up with team members.
• Collaborate with nursing, therapy, social services, and the interdisciplinary team.
• Develop and update comprehensive, resident-centered care plans.
• Monitor documentation to support regulatory compliance and appropriate reimbursement.
• Provide education to nursing staff on documentation standards and MDS requirements.
• Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
• Support survey preparation and ongoing survey readiness.
• Participate in quality improvement and clinical initiatives.
