Job summary

Eastcastle Place is seeking an experienced MDS Coordinator to lead the MDS process within our skilled nursing community in Milwaukee, WI. You will manage assessments, care planning, and regulatory readiness, coordinating with nursing, therapy, social services, and the interdisciplinary team to ensure accurate documentation and compliant reimbursement.

This deadline-driven, detail-oriented role offers flexible start/end times, 28–40 hours per week, and meaningful ownership of the MDS workflow

Qualifications

• Current, unrestricted RN license in Wisconsin.
• Minimum of 2 years of MDS experience in skilled nursing or long-term care.
• Strong working knowledge of the MDS assessment and care planning process.
• Experience managing assessment schedules and regulatory deadlines.
• Experience preparing for state surveys preferred.
• Strong clinical documentation review skills.
• Exceptional attention to detail and organizational skills.
• Ability to independently manage competing priorities while collaborating with the team.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments.
• Oversee the MDS assessment and care-planning process.
• Manage assessment schedules and submission deadlines.
• Review clinical documentation for completeness, accuracy, and consistency.
• Identify documentation gaps and follow up with appropriate team members.
• Collaborate with nursing, therapy, social services, and other interdisciplinary team members.
• Develop and update comprehensive, resident-centered care plans.
• Monitor documentation to support regulatory compliance and appropriate reimbursement.
• Provide education and guidance to nursing staff regarding documentation standards and MDS requirements.
• Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
• Support survey preparation and ongoing survey readiness.
• Participate in quality improvement and clinical initiatives.
• Stay current with applicable federal and Wisconsin MDS requirements and regulatory changes.
