Job summary

Eastcastle Place is seeking an experienced MDS Coordinator to manage assessments, care planning, and regulatory readiness. This role collaborates with nursing, therapy, social services, and other teams to ensure accurate documentation and timely reimbursement.

The successful candidate will own the MDS process, monitor deadlines, and support survey readiness while maintaining high standards of resident care. Wisconsin RN licensure is required.

Qualifications

• Current, unrestricted RN license in the state of Wisconsin.
• Minimum of 2 years of MDS experience in skilled nursing or long-term care.
• Strong working knowledge of the MDS assessment and care-planning process.
• Experience managing assessment schedules and regulatory deadlines.
• Experience preparing for and navigating state surveys preferred.
• Strong clinical documentation review skills.
• Exceptional attention to detail and organizational skills.
• Ability to independently manage competing priorities and deadlines.
• Strong communication skills and confidence working across departments.
• Ability to work independently while collaborating effectively with an interdisciplinary team.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments.
• Oversee the MDS assessment and care-planning process.
• Manage assessment schedules, documentation requirements, and submission deadlines.
• Review clinical documentation for completeness, accuracy, and consistency.
• Identify documentation gaps and follow up with appropriate team members.
• Collaborate with nursing, therapy, social services, and other interdisciplinary team members.
• Develop and update comprehensive, resident-centered care plans.
• Monitor documentation to support regulatory compliance and appropriate reimbursement.
• Provide education and guidance to nursing staff regarding documentation standards and MDS requirements.
• Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
• Support survey preparation and ongoing survey readiness.
• Participate in quality improvement and clinical initiatives.
• Stay current with applicable federal and Wisconsin MDS requirements and regulatory changes.
