Job summary

Nexion Health in Marrero, Louisiana seeks an MDS Coordinator to lead comprehensive resident assessments (MDS), ensure data accuracy, and support care planning with the interdisciplinary team. The role requires an LPN/LVN license and a nursing degree, along with knowledge of MDS regulatory standards.

You will collaborate closely with care teams, participate in orientation on MDS processes, and help ensure compliant reimbursement.

Qualifications

Must be a graduate of an accredited nursing program.
LPN/LVN license required.
Working knowledge of regulatory and compliance standards related to MDS.

Responsibilities

Coordinates and completes comprehensive resident assessments (MDS) and ensures accuracy and timeliness of assessment data.
Serves as a resource person for resident care planning activities and conducts orientation sessions on the MDS process.
Collaborates with the interdisciplinary team on care planning and supports regulatory compliance and reimbursement accuracy.
