Legacy Nursing in Louisiana is seeking a dedicated MDS/ Care Plan Nurse to ensure accurate and timely MDS assessments and to develop individualized care plans for residents.

You will collaborate with the interdisciplinary team, educate staff on documentation, and ensure compliance with Medicare/Medicaid rules while maintaining HIPAA standards. This full-time role requires an active LPN/LVN or RN license and experience in long-term care.

Qualifikationen

• Completion of an accredited nursing program.
• Active LPN/LVN or RN license (appropriate state).
• Experience in a long-term care setting preferred but not required.

• Complete and submit MDS assessments accurately and timely.
• Create and maintain individualized plans of care for each resident.
• Coordinate MDS assessments with the interdisciplinary team.
• Inform department management of due dates and ARD changes.
• Assist in training staff on MDS documentation.
• Maintain HIPAA compliance and professional conduct.
• Participate in QA and compliance activities.
