Job summary

Vintage Health Care Center in Denton, TX is seeking an experienced MDS Coordinator to ensure accurate resident assessments, regulatory compliance, and effective care planning. You will coordinate admissions, annuals, quarterlies, Medicare, and condition changes while collaborating with nursing and other departments.

The role requires strong organization, attention to detail, and ability to lead interdisciplinary meetings, update care plans, and provide quality reporting to leadership.

Qualifications

• Active Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Familiarity with nursing home regulations and compliance.
• Ability to work independently and in a team.
• Strong multi-tasking and accuracy.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure timely communication with nursing and other departments.
• Maintain MDS data accuracy and support care planning.
• Lead interdisciplinary meetings and coordinate care discussions.
• Update and revise care plans based on assessments.
• Generate and distribute Quality Indicators reports to leadership.
• Submit MDS data per reporting standards and timelines.
• Stay current with industry standards and best practices.
• Oversee ancillary data reporting to corporate billing.
