The role requires an active nursing license, geriatric care experience, strong assessment and documentation skills, and the ability to lead meetings and manage multiple priorities while maintaining accurate data entry in MDS software.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.
• Ability to work independently and in a team.
• Ability to manage multiple priorities with accuracy and efficiency.

Responsibilities

• Coordinate Resident Assessments: oversee admissions, annual, quarterly, Medicare, and change-of-condition assessments with timely communication to nursing and other departments.
• Ensure Regulatory Compliance: follow federal and state guidelines for MDS assessments.
• Monitor Documentation: review MDS and care plan files for accuracy and thoroughness.
• Utilize MDS Software: maintain proficiency in software systems for correct data entry and reporting.
• Lead Interdisciplinary Meetings: attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
• Update Care Plans: adjust and revise care plans based on current assessments and resident status.
• Provide Quality Reporting: generate and distribute Quality Indicators Reports to leadership.
• Submit MDS Data: ensure timely and accurate transmission of MDS assessments per reporting standards.
• Stay Current: attend professional development sessions to keep up with industry standards.
• Oversee Ancillary Data: collect and report ancillary services to corporate billing for accurate processing.
