Job summary

The Boulevard Post Acute is seeking an experienced MDS Coordinator to ensure timely, accurate MDS assessments and compliant resident documentation. This full-time, on-site role supports quality care, reimbursement, and regulatory compliance through collaboration with the DON and clinical teams.

Qualifications include an active RN license and at least three years in a long-term care setting, with prior MDS/RAI experience.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulatory requirements and deadlines.
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership team.
• Collaborate with interdisciplinary teams to ensure accurate resident care representation.
• Support audits and Medicare/Medicaid regulatory reviews.
