Job summary

Diversicare is seeking a dedicated MDS Coordinator (RNAC) to join our team and support accurate MDS assessments and compliant care planning. The role focuses on coordinating RAI processes, collaborating with the interdisciplinary team, and ensuring timely, accurate documentation.

Qualifications

• Two years of MDS experience preferred.
• Current RN license in the state of employment.
• Working knowledge of MDS 3.0.

Responsibilities

• Coordinate the RAI process with accuracy and regulatory compliance.
• Collaborate with the interdisciplinary team to assess patient needs and care plans.
• Conduct care plan conferences with patients, residents and families.
• Provide education on the RAI process and accurate MDS coding.
• Monitor quality measures and ensure MDS reflects quality standards.
• Maintain documentation and timely submissions to state databases and other entities.
• Ensure compliance with Medicare and Medicaid regulations.
