Job summary

Luna Rehabilitation & Care Center in Roswell, NM seeks an experienced MDS Coordinator to join our clinical leadership team. You will oversee the RAI process, ensure timely MDS assessments, care planning, and regulatory compliance while optimizing reimbursement.

You’ll collaborate with nursing leadership, therapy, social services, and dietetics to maintain resident-centered care, monitor QM measures, and contribute to QA/PI initiatives. Excellent attention to detail is essential.

Qualifications

• RN or LPN license in good standing in NM.
• Strong knowledge of CMS regs, RAI, PDPM, and Medicare reimbursement.
• Experience with care planning and interdisciplinary coordination.

Responsibilities

• Conduct CMS-compliant MDS assessments per requirements.
• Coordinate and submit MDS assessments within regulatory timelines.
• Establish ARDs and manage MDS scheduling.
• Develop and update individualized resident care plans.
• Collaborate with interdisciplinary teams to reflect resident needs.
• Monitor changes and update MDS and care plans accordingly.
• Maintain accurate documentation for reimbursement and regulatory purposes.
• Support survey preparation and regulatory compliance.
