Job summary

Roswell Center for Nursing and Healing is seeking an MDS Nurse Coordinator (RN or LPN) to oversee RAI/MDS processes and ensure CMS/LTCI compliance at the Roswell, GA facility. The role emphasizes interdisciplinary collaboration and accurate MDS submissions.

The MDS Coordinator will lead care plan meetings, track QM/QI indicators, and contribute to performance improvements while supporting staff education and regulatory adherence.

• Must be a RN or LPN.

Responsibilities

• Initiates and actively participates in Interdisciplinary Care Plan meetings.
• Coordinates the development and revision of resident Interdisciplinary Care Plans based on information identified in the RAI/MDS.
• Ensures the Interdisciplinary Care Plan addresses all triggered indicators.
• Collaborates with nursing caregivers and other disciplines to obtain information on residents' status and progress toward care plan goals.
• Communicates effectively with a variety of other disciplines to ensure accurate and timely completion of the RAI/MDS for transmission.
• Prepares accurate reports of QM/QI derived from the RAI/MDS which drives data for the CLC Compare; tracks and trends data for quality improvement.
• Active involvement in Quality Improvement.
• Implements an educational plan to meet changing program or service needs for self and others.

Summary:

Responsible and accountable to the Administrator. The MDS Coordinator Serves as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for the Center. Duties include but are not limited to reviewing and processing MDS assessments for submission per CMS and LTCI regulations and guidelines.

• Demonstrates the ability to communicate effectively with a variety of other disciplines to ensure the accurate and timely completion of the RAI/MDS for transmission.
• Prepares accurate reports of the Quality Measure/Quality Indicators (QM/QI) derived from the RAI/MDS which drives the data for the CLC Compare. Performs tracks and trends of this data to enhance the knowledge of the Interdisciplinary Team and improve quality of care through performance improvement initiatives.
• Active involvement in Quality Improvement.
• Implements an educational plan to meet changing program or service needs for self and others.
