Marquis Health Consulting Services in Springfield, MA seeks an experienced MDS Coordinator to manage MDS assessments, care planning, and regulatory compliance in a long-term care facility.

The role requires 2–3 years nursing experience, strong knowledge of OBRA/Medicare, and proficiency with EHR and MDS software. This full-time position offers up to $50 hourly all-inclusive. You will lead interdisciplinary teams and support audits to ensure high-quality care.

• Nursing experience: 2–3 years in general nursing (LPN or RN).
• Clinical knowledge: strong understanding of assessment, care planning, and regulations.
• Technical skills: proficiency in data entry, EHR, and MDS software systems.

Responsibilities

• Assessment Management: complete MDS forms for residents (initial, quarterly, annual, and changes).
• Care Planning: collaborate with multidisciplinary teams to update individualized plans.
• Regulatory Compliance: ensure OBRA/Medicare guidelines and data submission.
• Data Collection: conduct interviews, perform assessments, review records for accuracy.
• Team Coordination: lead interdisciplinary meetings and educate staff on MDS standards.
• Resident & Family Interaction: explain care plans and address questions/concerns.
• Quality Monitoring: track progress, update plans, and support audits.
