Job summary

The Oxford Rehab is seeking an MDS Coordinator to oversee the MDS process, care planning, and compliance at our 120-bed skilled nursing facility. The role focuses on coordinating assessments, documentation, and CMS MDS item sets to ensure high-quality resident care.

Ideal candidates hold a Nursing degree with RN preferred and must have an active RN/LPN license in the state. Competitive pay and full benefits accompany this role.

Qualifications

• Must have a Nursing Degree from an accredited school; RN preferred.
• Must hold a current, unencumbered RN or LPN license in the state.

Responsibilities

• Coordinate MDS process, including resident assessment, care planning, and data collection.
• Oversee ADL training for facility and staff.
• Issue and deliver denial notices timely and appropriately.
• Complete and transmit all CMS-approved MDS item sets.
• Maintain adequate systems to support billed services.
