Job summary

Pinnacle Point Wellness & Rehabilitation is seeking a registered or licensed practical nurse to lead MDS coordination and care management in a skilled nursing facility. You will drive timely MDS submissions, plan care, and collaborate with the interdisciplinary team to ensure compliant documentation.

In this role you will monitor regulatory requirements, maintain data accuracy, and support continuous quality improvement under the direction of the DON and clinical leadership.

Qualifications

• Graduate of an approved Licensed Vocational program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and Minimum Data Set.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and coordinate MDS schedules.
• Ensure decisions on MDS and assessments align with clinical criteria and regulatory guidelines.
• Assist with daily stand-up meeting coordination and set assessment reference dates.
• Comply with federal and state regulations for MDS processes and documentation.
• Monitor MDS/care plan documentation in the medical record for accuracy.
• Maintain current MDS status per guidelines and ensure timely data entry in MDS systems.
• Code MDS accurately from chart reviews and interviews.
• Attend interdisciplinary and quality meetings to communicate changes and update records.
• Assist DON with identifying significant changes and ensure MDS reflects changes.
