Job summary

Windsor Estates of St. Charles is seeking an MDS Coordinator / LPN to ensure timely and accurate RAI and care management from admission through discharge.

You will work under the Director of Nursing and VP of Clinical Reimbursement to coordinate clinical information systems and staff education. The role requires an active LPN license, MDS experience, and strong knowledge of Case-Mix, PPS, and Medicaid reimbursement, with excellent communication and patient-centered care.

Qualifications

• Licensed as an LPN in the state of practice.
• Prior MDS experience is highly preferred.
• Experience in LTC/SNF/AL/MC settings is preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding OBRA regulations and MDS required.
• Knowledge of the care planning process.
• Professional image in appearance and behavior.
• Excellent written and oral communication skills.

Responsibilities

• Complete the RAI and care management processes from admission to discharge.
• Coordinate information systems operations and education for the clinical department.
• Track admissions, discharges, and data related to MDS and care plans.
