Fundamental is seeking an MDS Nurse (LPN) in Henderson, NV to coordinate the RAI process and ensure accurate MDS completion for the Interdisciplinary Plan of Care. This role supports the DON and nursing leadership in a patient-centered service environment.

The position requires an active LPN license and 1+ years of direct nursing with MDS/RAP experience; you will collaborate with the ITM and healthcare team to ensure compliance and quality care, with a competitive full-time package.

Qualifications

• Must have a current, unencumbered LPN license (as required by state).
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
• Ability to utilize both local and consultant resources in the execution of job responsibilities.
• Must have the ability to develop knowledge of state, federal, and regulatory requirements related to the job.
• Reliable transportation required.

Responsibilities

• Coordinating the Resident Assessment Instrument (RAI) process.
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
• Participating in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establishing and maintaining positive relationships with all members of the healthcare team and will function as part of the nursing management team.
