Job summary

Highfield Nursing and Rehabilitation in Cary, NC is seeking a qualified MDS Coordinator (RN or LPN) to oversee the RAI process, ensure accurate assessments, and support compliant care planning and reimbursement.

The role requires a current RN or LPN license and at least 6 months of MDS experience; RAC-CT or RNac certification is preferred. You will collaborate with the interdisciplinary team to maintain high-quality resident care.

Qualifications

• Active RN or LPN license in good standing.
• 6+ months of experience as an MDS Coordinator.
• RAC-CT or RNAC certification preferred.
• Experience in Skilled Nursing/Rehabilitation preferred.

Responsibilities

• Coordinate and participate in the completion of the MDS and Care Plans in compliance with Federal and State regulations.
• Monitor and document the management of Medicare and Managed Care residents with the interdisciplinary team.
• Ensure accuracy in resident assessments to maximize reimbursement and quality of care.
• Drive innovation by implementing new ideas and processes to improve resident outcomes.
• Maintain compliance with evolving regulations and best practices.
