Responsibilities

• Coordinate and facilitate development of the Minimum Data Set (MDS) for each resident, ensuring accuracy, completeness, and regulatory compliance.
• Collaborate with interdisciplinary team to identify resident needs and develop individualized care plans.
• Conduct thorough assessments to identify resident goals and outcomes.
• Maintain accurate records of resident assessments, care plans, and progress.
• Develop and implement strategies to improve quality of care and resident outcomes.
• Participate in facility-wide quality improvement initiatives and projects.
• Maintain knowledge of relevant regulations, policies and procedures.
