Job summary

Creative Solutions In HealthCare seeks an experienced MDS Coordinator to ensure accurate assessments and compliant care planning for residents. The role emphasizes reliability, organization, and collaboration across disciplines to optimize resident care.

You will oversee MDS assessments, maintain data integrity in the software systems, and support leadership with quality reporting. A current RN license and geriatric nursing experience are essential for success in this position.

Qualifications

• Must hold a current RN license in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance requirements.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change of condition).
• Ensure timely communication with nursing and other departments.
• Maintain compliance with Federal and State guidelines for MDS.
• Review MDS and care plan files for accuracy and organization.
• Maintain expertise in MDS software for data entry and reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Ensure timely submission of MDS data per reporting standards.
• Stay current through professional development.
