Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center seeks an MDS Coordinator who is detail oriented, organized, and self-motivated to support resident assessments and regulatory compliance. Knowledge of nursing practices and MDS processes is essential.

You will work closely with the interdisciplinary team to complete accurate MDS data, participate in PPS and Medicare meetings, and ensure care plans reflect quarterly changes. A current nursing license and MDS experience are required.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current state licensure.
• Minimum three years of clinical experience in a health care setting; long-term care experience preferred.
• Minimum one year of MDS experience.

Responsibilities

• Collect and complete the MDS using medical records, bedside assessments, and interviews with staff, residents, and families.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end meetings to ensure federal billing requirements are met; review Plan of Care quarterly and with Comprehensive Assessments.
• Educate the interdisciplinary team on the RAI process and changes as appropriate.
