Job summary

Augusta Nursing and Rehabilitation is seeking an experienced MDS Coordinator (RN or LPN) to oversee the Resident Assessment Instrument process, MDS coordination, and care planning in a skilled nursing setting.

Candidates should have 6+ months as an MDS Coordinator, RN or LPN licensure, and a focus on accuracy and compliance. RAC-CT or RNAC certification is preferred, with comprehensive benefits and scheduling flexibility.

Qualifications

• Active RN or LPN license in good standing.
• 6+ months experience as MDS Coordinator.

Responsibilities

• Coordinate and participate in completing the MDS and care plans in compliance with regulations.
• Monitor Medicare and Managed Care resident documentation with the interdisciplinary team.
• Ensure accuracy in resident assessments to maximize reimbursement and care quality.
• Drive process improvements to support resident outcomes and regulatory updates.
• Maintain compliance with evolving regulations and best practices.
