Job summary

Topaz Fiscal Services seeks an MDS Coordinator to oversee the timely completion of the MDS process from admission to discharge. You will collaborate with the facility team to ensure accurate documentation and compliant coding, while maintaining schedules and data entry across electronic systems.

Ideal candidates hold Kentucky RN licensure with at least two years of MDS experience in long-term care and RAC-CT certification or a plan to obtain it within 90 days of hire.

Qualifications

• Must have current RN licensure in Kentucky and at least two years of MDS experience in long-term care.
• AANAC RAC-CT certification or intent to obtain within 90 days of hire.

Responsibilities

• Collaborate with facility team to assess resident needs.
• Coordinate care management meetings and review resident care and assessment dates.
• Ensure compliance with federal and state MDS regulations.
• Monitor MDS and care plan documentation for all residents.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data accurately in appropriate computerized systems.
