Job summary

Topaz Fiscal Services is seeking a strong MDS Coordinator to ensure timely and accurate completion of the MDS process from admission to discharge.

The role requires RN licensure in Kentucky and at least two years of MDS experience in long-term care; LVN/LPN may be considered with exceptional qualifications or equivalent education/experience.

Qualifications

• RN licensure in the state with 2+ years MDS experience in long-term care.
• LVN/LPN considered with exceptional qualifications; or equivalent education/experience.
• Ability to coordinate information systems and education for the clinical department.

Responsibilities

• Collaborate with facility team to assess resident needs.
• Assist with coordinating care management meetings and set assessment reference dates.
• Ensure compliance with federal and state regulations for MDS completion.
• Monitor MDS and care plan documentation for all residents.
• Maintain MDS status and data entry in appropriate computerized programs.
• Complete accurate coding of MDS using medical records and interviews.

We are looking for a strong individual to fill our MDS Coordinator position.

RN licensure within the state plus a minimum of two years MDS experience in long term care Preferred. LVN or LPN may be considered with exceptional qualifications; or equivalent combination of education and experience.

Position Purpose:

Responsible for the timely and accurate completion of the MDS process from admission to discharge. May coordinate information systems, operations and education for the clinical department.

Essential Functions of Postition:

• Works in collaboration with facility team to assess the needs of the resident
• Assist with coordination and management of care management meetings, to include review of residents care and the setting of assessment refernce dates.
• Complies with federal and state regulations reguarding completion and coordination of the MDS process.
• Monitors MDS and care plan documentation for all residents.
• Ensures documentation is present in the medical record to support MDS coding.
• Maintains current MDS status of assigned residents according to state and federal guidelines.
• Supports the tracking system of MDS schedules (timeframes and due dates)
• Maintains the frequent and accurate data entry of residents information into appropriate computerized programs.
• Completes accurate coding of the MDS with information obtained via medical record reveiw as well as observation and interview with facility staff, resident and family members.
• As required, attends team meetings and other meetings in order to gather information, communicate changes and maintain and update records.
• Continually updating knowledge base related to data entry and computer technology.
• As required, may participate in quality assurance activities.
• May complete eletronic submission of required documentation to required databases and other entities per company policy.
• Perform addition duties as assigned.
