Job summary

Southern Healthcare Management is seeking an LPN with MDS/RDA experience to join our team as an MDS Coordinator. You will develop, implement and coordinate the RAI process, ensuring compliance with OBRA, Medicare/Medicaid, and facility policies.

Qualifications

• Active, unrestricted LPN licensure in the state of practice.
• Working knowledge of Medicare/Medicaid programs, OBRA, and MDS/RAI processes preferred.

Responsibilities

• Develop, implement and coordinate the RAI/MDS process to ensure compliance with professional standards and regulations.
• Ensure adherence to physician orders, company policies and local, state, and federal regulations.
• Support ongoing improvement of patient assessment workflows and care planning.

As an MDS Coordinator, you will be responsible for developing, implementing and coordinating the Resident Assessment Instrument (RAI) process, ensuring the center stays in compliance with current professional practice standards, physician's orders, company policies and procedures, and local, state, and federal regulations.

Entry Qualifications:

• Active, unrestricted Licensed Practical Nurse LPN licensure in the state of practice required
• Working knowledge of Medicare and Medicaid programs, LTC payer systems, the Omnibus Budget Reconciliation Act (OBRA), the RAI process, and the MDS process preferred OR prior nursing in a long-term care facility
