Job summary

Apploi is hiring an experienced RN or LPN to serve as an MDS Coordinator. You will ensure timely completion of the RAI process and care management from admission to discharge in a skilled nursing setting.

Collaborate with the Interdisciplinary Team to develop MDS assessments and care plans, document accurately, stay current with MDS 3.0, Medicare PPS, Medicaid and OBRA regulations, and submit data to state and federal agencies.

Qualifications

• Graduate of an RN or LPN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• In-depth knowledge of MDS 3.0, Medicare PPS, Medicaid, and OBRA regulations.
• Experience with care planning and quality indicators.

Responsibilities

• Ensure accurate and timely documentation of the MDS process, including coding and interdisciplinary updates.

• Submit resident data to state and federal agencies, and ensure all MDS documentation is accurate and complete.

What We're Looking For:

• Graduate of an approved Registered Nurse (RN) or Licensed Practical Nursing (LPN) program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
• In-depth knowledge of the MDS 3.0, Medicare PPS process, Medicaid reimbursement, and OBRA regulations.
• Experience with care planning and quality indicators.
• Strong attention to detail and excellent communication skills.
