Job summary

Aspire Senior Living Pleasant Hill is seeking a dedicated LPN/RN MDS Coordinator to join our team. In this role you will coordinate MDS assessments, collaborate with the interdisciplinary team, and develop personalized care plans to enhance resident well-being.

Qualifications

Responsibilities

• Coordinate and implement MDS assessments and the Masterpiece Living initiative.
• Collaborate with Interdisciplinary Team Members to develop care plans.
• Provide exceptional customer service to residents, families, and staff.
• Maintain accurate records and leverage technology to improve efficiency.
• Participate in quality improvement initiatives and staff education.

Responsibilities:

• Coordinate and implement the Masterpiece Living for Individuals initiative, ensuring thorough and accurate completion of MDS assessments
• Collaborate with Interdisciplinary Team Members to develop and implement individualized care plans, addressing resident needs and preferences
• Provide exceptional customer service, ensuring prompt and courteous communication with residents, families, and staff members
• Maintain accurate and up-to-date records, leveraging technology to streamline processes and enhance efficiency
• Participate in quality improvement initiatives, driving continuous growth and improvement in our care services
• Develop and present educational programs, workshops, and training sessions to enhance staff knowledge and skills

Requirements:

• Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in that state of Missouri
• Experience in a long-term care setting or related field preferred
• Strong understanding of MDS assessment and care planning principles
• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
• Excellent communication, interpersonal, and problem-solving skills
• Ability to maintain confidentiality and handle sensitive information with discretion
