Job summary

Bella Terra Lombard is seeking a dedicated MDS Coordinator to join our compassionate team. You will develop, monitor, audit, and modify each resident’s care plan to meet individual goals and needs, and you will perform resident assessments and assist in the discharge process.

• CPR certification required.
• Prior MDS Coordinator experience preferred; RN in Bella Terra Lombard setting and CNA experience also preferred.

As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Bella Terra Lombard by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
To be eligible for consideration applicants should have:
As a minimum, an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Bella Terra Lombard setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
