Job summary

Chicago Skilled Nursing in Chicago, IL is seeking an experienced MDS Coordinator to join our admissions and care planning team. You will lead resident assessments, develop, monitor, and audit individualized care plans, and support the discharge process to ensure high-quality outcomes.

The ideal candidate holds an Illinois RN license, CPR certification, and at least two years of MDS Coordinator and RN SNF experience.

Qualifications

• Unencumbered Illinois RN license and nursing degree required.
• CPR certification required; two years of MDS Coordination experience preferred.
• Two years RN experience in a Skilled Nursing Facility setting.
• One year of CNA experience is a plus.

Responsibilities

• Develop, monitor, audit and modify each resident's care plan.
• Perform resident assessments and assist in the discharge process.
• Ensure residents receive high-quality care and maintain standard practices.

As the MDS Coordinator, you are instrumental in giving your team the knowledge they need to care for each resident's unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process.

To be eligible for consideration applicants must have:

• As a minimum, an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program;
• C.P.R. Certification;
• at least two (2) years of prior experience as a MDS Coordinator;
• two (2) years of experience as an R.N. in a Skilled Nursing Facility setting;
• and (1) year of experience as a C.N.A..
