Sapphire at Maple Crest in Belvidere, IL is seeking an MDS Coordinator to support the Director of Nursing in ensuring documentation meets Federal, State, and Certification guidelines. The role coordinates the RAI process, MDS 3.0, CAAs, and the interdisciplinary care plan.

Ideal candidate will have a nursing degree and an active state license with at least 2 years of SNF experience and solid knowledge of Medicare/Medicaid processes. Join our supportive team in a post-acute setting.

Qualifications

• Graduate of an approved nursing program and licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of MDS, OBRA, and Medicare/ Medicaid processes.

Responsibilities

• Complete the RAI process from MDS to care plan.
• Initiate and monitor RAI tracking in Point Click Care.
• Oversee MDS 3.0 regulatory and financial tasks.
• Ensure timely completion and submission; review validation reports.
• Follow up to ensure documentation standards.
• Conduct patient assessments and data collection.
• Determine Assessment Reference Date reflecting care needs.
• Provide interdisciplinary schedules for MDS assessments and OBRA/PPS.
• Obtain necessary signatures.

Description:

The primary purpose of the MDS Coordinator is to assist the Director of Nursing while ensuring that documentation in the center meets Federal, State, and Certification guidelines. The MDS Coordinator coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.

Responsibilities for MDS Coordinator:

• Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the Point Click Care system.
• Oversees functions related to the MDS 3.0 both regulatory and financial including scheduling, enduring timeliness in completion and submission, CAA and comprehensive care plan, reviewing and correcting validation reports, monitoring for significant changes and IPA.
• Follows up with staff when necessary to assure compliance to standards of documentation.
• Completes patient assessments, data collection, and interviews staff as necessary to assure good standard of practice and as instructed in the current version of MDS User’s Manual.
• Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient’s care needs and captures all resources utilized to ensure appropriate payment by Medicare/Medicaid and insurance programs.
• Provides interdisciplinary schedules for all MDS assessments and care plans as required by OBRA and PPS.
• Assures that appropriate signatures are obtained as required.

Qualifications for MDS Coordinator:

• Graduate of an approved Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case- Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Through understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. Knowledge of the care planning process.
