Job summary

Legacy Healthcare seeks an experienced MDS Coordinator RN to support a premier skilled nursing and rehabilitation facility in Illinois. You will develop, monitor, and modify care plans, perform assessments, and assist with discharge to ensure high-quality, individualized resident care.

Qualifications

• Illinois RN license is required.
• Graduate of an accredited nursing program is required.
• CPR certification required.
• Prior experience as an MDS Coordinator is preferred.
• At least 1 year RN experience in a skilled nursing setting.
• At least 1 year CNA experience.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plans.
• Perform resident assessments and assist in the discharge process.
• Ensure residents receive high-quality care and safety across the facility.

As an MDS Coordinator, you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

• As a minimum, an unencumbered State of Illinois R.N. License;
• A graduate of an accredited nursing program
• C.P.R. Certification.
• Prior experience as a MDS Coordinator;
• One (1) year of experience as an R.N. in a skilled nursing setting
• One (1) year of experience as a C.N.A.
