Qualifications

  • Graduate of an accredited nursing program with RN or LPN licensure.
  • One year experience in long-term care and knowledge of MDS process.

Your Role & Responsibilities:

  • Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned.
  • Conducts daily rounds and assessments to develop and update care plans.
  • Conducts care plan conferences for assigned residents.
  • Maintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary.
  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.

Qualifications:

  • Graduate of an accredited school of nursing in preparation for Registered Nurse or Licensed Practical Nurse licensure.
  • Registered Nurse or Licensed Practical Nurse currently licensed by the Kentucky Board of Nursing.
  • One year previous experience in long term care environment.
  • Knowledge of MDS and Care Plan process.
  • Basic knowledge of PPS and RUGs.
  • Ability to work both independently and collaboratively.
