Your Job Summary

The MDS Coordinator will be responsible for the timely and accurate completion of both the RAI process and care management process from admission to discharge, in accordance with company policies, procedures, and federal, state, and certification guidelines. This includes tasks such as Minimum Data Set, discharge and admission tracking, etc. Under the direction of the Director of Nursing and VP of Clinical Reimbursement, the coordinator may also oversee information systems operations and staff education within the clinical department.

Principal Responsibilities
  1. Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
  2. Ensure the interdisciplinary team makes informed decisions regarding additional MDS assessments based on clinical criteria outlined in the latest RAI User’s Manual.
  3. Assist with coordinating and managing daily stand-up meetings, including reviewing resident care and setting assessment reference dates.
  4. Comply with federal and state regulations concerning the RAI process.
  5. Monitor MDS and care plan documentation for all residents, ensuring proper documentation supports MDS coding.
  6. Maintain current MDS status for assigned residents following applicable guidelines.
  7. Accurately enter resident data into computerized MDS programs regularly.
  8. Code MDS accurately by reviewing medical records and consulting with staff, residents, and families.
  9. Participate in interdisciplinary meetings, quality assurance, and other meetings to gather information, communicate updates, and maintain records.
  10. Assist with identifying significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect current resident status.
  11. Prepare scheduling notices for care planning conferences and communicate outcomes to staff, residents, and families.
  12. Continuously update knowledge related to data entry and computer technology.
  13. Complete electronic submissions of documentation to state databases and other entities as per policy.
  14. Ensure final MDS completion and submit resident assessment data to appropriate agencies.
  15. Instruct staff on the RAI process, PPS Medicare, Medicaid, and related clinical computer systems.
  16. Maintain confidentiality of all necessary information.
  17. Perform other duties as assigned, which may change over time.
Qualifications
  1. Graduate of an approved RN program and licensed in the state of practice.
  2. Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
  3. Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement processes.
  4. Thorough understanding of Quality Indicators, OBRA regulations, and MDS 3.0.
  5. Experience with care planning and MDS coding.
