Job summary

Cedars of Lebanon is seeking an experienced MDS Coordinator (RN or LPN) to join our long-term care team. This full-time role focuses on coordinating the RAI process, ensuring timely MDS submissions, and supporting care planning across interdisciplinary teams.

Responsibilities include supervising MDS input, coordinating state transmission, and leading care plan conferences. A RAC-CT certification is preferred, with opportunities for professional growth and competitive benefits.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• Current, unencumbered RN or LPN license and CPR.
• At least 3 years' experience in Long Term Care as a MDS Coordinator.
• RAC-CT preferred.

We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon, As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.

Responsibilities:

Mandatory Qualifications:
