Job summary

Legacy Health and Rehab LLC in West Meade, TN seeks an experienced MDS Coordinator to ensure accurate resident assessments, individualized care plans, and regulatory compliance within our skilled nursing team.

You will collaborate with nursing, therapy, social services, and dietary staff to complete timely MDS submissions, support PDPM classification and reimbursement, and participate in audits, quality-improvement initiatives, and staff education.

Qualifications

• Current Tennessee nursing license in good standing.
• Registered Nurse preferred; LPN candidates will be considered.
• Experience with MDS in skilled nursing or long-term care is preferred.
• Knowledge of RAI, PDPM, Medicare requirements and long-term care regulations.
• Strong organizational, communication, and time-management skills.
• Experience with electronic health records and MDS software.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments per CMS and state requirements.
• Manage the MDS schedule and ensure timely completion of assessments.
• Coordinate resident care-planning meetings and develop person-centered care plans.
• Review documentation for accuracy and clinical support.
• Collaborate with interdisciplinary team on resident needs and interventions.
• Monitor Medicare, Medicaid, and managed-care requirements.
• Support accurate PDPM classification and reimbursement.
• Track quality measures and assist with quality-improvement initiatives.
• Participate in audits, surveys, utilization reviews, and documentation reviews.
• Educate staff on documentation practices and MDS requirements.
• Maintain compliance with federal, state, and facility policies and regulations.

Job description

Legacy Health and Rehab is seeking an experienced, organized, and dependable MDS Coordinator to join our skilled nursing team. This position plays an essential role in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement.

The ideal candidate will have strong clinical judgment, excellent attention to detail, and the ability to collaborate effectively with nursing, therapy, social services, dietary, and other members of the interdisciplinary team.

Responsibilities:

• Coordinate and complete accurate, timely MDS assessments in accordance with CMS and state requirements
• Manage the MDS schedule and ensure all assessments are completed within required timeframes
• Coordinate resident care-planning meetings and maintain individualized, person-centered care plans
• Review resident documentation for accuracy, completeness, and clinical support
• Collaborate with the interdisciplinary team to identify resident needs, changes in condition, and appropriate interventions
• Monitor Medicare, Medicaid, and managed-care requirements
• Support accurate PDPM classification and reimbursement
• Track quality measures and assist with quality-improvement initiatives
• Participate in audits, surveys, utilization reviews, and documentation reviews
• Educate staff regarding documentation practices and MDS-related requirements
• Maintain compliance with all federal, state, and facility policies and regulations

Qualifications:

• Current Tennessee nursing license in good standing
• Registered Nurse preferred; qualified LPN candidates will be considered
• Previous MDS experience in a skilled nursing or long-term care setting strongly preferred
• Knowledge of the RAI process, PDPM, Medicare requirements, and long-term care regulations
• Strong organizational, communication, and time-management skills
• Excellent attention to detail and ability to meet firm deadlines
• Experience with electronic health records and MDS software
• Ability to work collaboratively as part of an interdisciplinary team
