Job summary

Cedars of Lebanon is seeking a full-time MDS Coordinator to manage the RAI process, ensure timely MDS submissions, and coordinate care plans with the interdisciplinary team.

The role requires an active RN or LPN license with CPR and minimum 3 years in long-term care as an MDS Coordinator. RAC-CT is preferred. Competitive wages and benefits are offered as part of our supportive workplace.

MDS Coordinator- RN OR LPN

We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon, As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.

Responsibilities

• Monitor and guide the completion of assessments in a timely manner
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtaining receipt of validation
• Schedule and facilitate care plan conferences and monthly QM meetings
• Manage Care Planning Process

Mandatory Qualifications

• Must hold a Nursing Degree from an accredited college or university
• Current, unencumbered, active RN license or LPN and CPR
• At least 3 years' experience in Long Term Care as a MDS Coordinator
• RAC-CT Preferred

Desired Skills and Qualities

• Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
• Excellent written and verbal communication skills and critical thinking abilities
• Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
