Job summary

Cedars of Lebanon is seeking a full-time MDS Coordinator to manage the RAI process, ensure timely MDS submissions, and coordinate care plans with the interdisciplinary team.

The role requires an active RN or LPN license with CPR and minimum 3 years in long-term care as an MDS Coordinator. RAC-CT is preferred. Competitive wages and benefits are offered as part of our supportive workplace.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• Current, unencumbered RN or LPN license with CPR.
• At least 3 years in Long Term Care as an MDS Coordinator.

Responsibilities

• Monitor and guide the completion of assessments in a timely manner.
• Inputting MDS assessments, transmission to state, and validation receipt.
