Job summary

Lebanon South Nursing & Rehab in Lebanon, Missouri is seeking an MDS Coordinator to join our care team. The role focuses on accurate MDS assessments, care plans, calendars, and ensuring timely documentation.

The position requires an active RN or LPN license, MDS experience in long-term care, and strong organizational and communication skills. You will collaborate with the DON and participate in QAPI and Medicare-related committees. 40-hour week, on-site role with a supportive team.

Qualifications

• Must hold an active RN or LPN license.
• Experience with MDS processes and care planning.
• Knowledge of LTC survey regulations preferred.
• Strong organizational and time-management abilities.
• Excellent communication and teamwork.
• 40-hour per week availability.

Responsibilities

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned


• Initiating care plans and supporting activities as assigned


• Creating and distributing monthly care plan calendars in a timely fashion


• Maintaining & updating all care plans and assessments as required


• Monitoring & auditing clinical records, ensuring accuracy & timeliness


• Informing DON of persistent issues related to non-compliant documentation


• Protecting the confidentiality of Resident & Facility information at all times

MDS Coordinator Qualifications and Skills:

• Valid RN or LPN License


• Knowledge of skilled nursing facilities survey process and regulations would be helpful.


• Must have prior MDS Experience


• Long Term Care Experience Required


• Must be able to work 40 hours a week


• Must be highly organized, professional & motivated


• Should have solid computer skills
