Job summary

Caretree Health Corporate is seeking an RN MDS Coordinator to assure timely completion of accurate MDS assessments in compliance with Federal and State guidelines.

This role leads MDS assessors, coordinates care plan meetings, ensures accurate diagnoses are entered in the EMR, and supports interdisciplinary teamwork. A NYS RN license and availability 9-5 Monday–Friday are required. Competitive benefits and growth opportunities offered.

Qualifications

• Registered Nurse with NYS Nursing License.
• Experience with MDS processes and data analysis preferred.
• Must be available Monday through Friday 9-5.

Responsibilities

• Oversee MDS assessors and ensure compliance with state and federal guidelines.
• Coordinate care plan meetings and ensure accurate diagnoses in the EMR.
• Lead interdisciplinary team meetings and support utilization reviews.

Description:

• The RN MDS Coordinator is responsible for assuring the timely completion of accurate MDS assessments that are in compliance with Federal and State guidelines.

• The RN MDS Coordinator is a team leader responsible for overseeing MDS assessors, making sure all aspects of the MDS are in compliance with state and Federal guidelines such as, scheduling care plan meetings, ensuring accurate diagnosis are placed in the EMR, certifications, Advance Beneficiary Notices, MDS Submissions, an attending morning huddle and Utilization Review meetings.

• The RN MDS Nurse is responsible to ensure accurate scores are obtained via the MDS to accurately reflect the residents and communication with the interdisciplinary team.

Requirements include:

• Registered Nurse with a current NYS Nursing License

• Must meet the requirements of the Department of Health

• Must be available Monday through Friday 9-5
