Job summary

Newfane Rehab and Health Care Center is seeking an RN MDS Coordinator to ensure timely, accurate MDS assessments in compliance with federal and state guidelines. You will lead a team of MDS assessors and coordinate with the interdisciplinary team.

Requirements include NYS Registered Nurse license, DOH eligibility, and availability Monday–Friday 9-5. We offer competitive pay, great benefits, professional growth, and ongoing training.

Qualifications

• RN MDS Coordinator responsible for ensuring timely, accurate MDS assessments in compliance with Federal and State guidelines.
• LEADS a team of MDS assessors and coordinates with the interdisciplinary team to ensure all aspects of MDS are compliant.

Responsibilities

• Oversee MDS assessments and ensure accurate scores reflecting residents.
• Schedule care plan meetings and ensure diagnoses are properly documented in EMR.
• Attend morning huddles and Utilization Review meetings to drive quality and compliance.

Description:

• The RN MDS Coordinator is responsible for assuring the timely completion of accurate MDS assessments that are in compliance with Federal and State guidelines.

• The RN MDS Coordinator is a team leader responsible for overseeing MDS assessors, making sure all aspects of the MDS are in compliance with state and Federal guidelines such as, scheduling care plan meetings, ensuring accurate diagnosis are placed in the EMR, certifications, Advance Beneficiary Notices, MDS Submissions, an attending morning huddle and Utilization Review meetings.

• The RN MDS Nurse is responsible to ensure accurate scores are obtained via the MDS to accurately reflect the residents and communication with the interdisciplinary team.

Requirements include:

• Registered Nurse with a current NYS Nursing License

• Must meet the requirements of the Department of Health

• Must be available Monday through Friday 9-5
