Job summary

Williamsville Suburban seeks an experienced full‑time MDS Coordinator to oversee the entire patient assessment process and ensure timely, accurate assessments and care plans. RAC‑CT preferred; MDS certification and PDPM/OBRA knowledge are required.

Detail oriented, able to work independently while maintaining high accuracy; training available for qualified candidates. The position offers four days on site and one day remote, competitive salary with full benefits and a $5,000 signing bonus after

Qualifications

• Active RN license from New York.
• MDS Coordinator with RAC-CT preferred.
• Knowledge of PDPM and OBRA regulations pertaining to MDS.
• Detail oriented and able to work independently with accuracy.

Responsibilities

• Oversee the patient assessment process and ensure assessments and care plans are completed accurately and on time.
• Develop, implement and monitor care plans based on assessments.
• Coordinate with interdisciplinary team to optimize resident outcomes.

• We are paying a $5,000.00 signing bonus for this position (to be paid upon successful completion of a 90 day arbitration period);
• This position’s schedule requires four days on site work and will allow for one day remote work;

Williamsville Suburban, is seeking an experienced full‑time MDS Coordinator. The MDS Coordinator is responsible for overseeing the entire patient assessment process and ensuring that assessments and care plans are done accurately and in a timely manner. The successful applicant will be RAC – CT (preferred), MDS certified and have knowledge of PDPM and OBRA regulations pertaining to MDS. The qualified applicant should be detail oriented and able to work independently in a manner that is accurate and concise. We are looking for experienced candidates but will train qualified candidates.

Requirements:

• Active RN license from the State of New York.
