Job summary

The MDS Coordinator at South Carolina Health Care Association is responsible for the accurate and timely completion of MDS assessments for residents, coordinating comprehensive care planning, and ensuring regulatory compliance to optimize reimbursement.

The role requires RN licensure in SC, a strong background in MDS 3.0 guidelines, and proficient use of EHR/MDS software. Collaboration with the interdisciplinary team and leadership in care planning are essential.

Qualifications

• Associate’s or Bachelor's degree in Nursing (ADN or BSN) or related healthcare field.
• Valid RN licensure in SC.
• At least 5 years of experience in a skilled nursing facility with MDS coordination.
• In-depth knowledge of MDS 3.0 guidelines.
• Strong assessment, communication, and organizational skills.
• Proficiency in EHR and MDS software.
• Detail-oriented with good time-management.

Responsibilities

• Oversee and manage the MDS assessment process (initial, quarterly, annual, and significant change).
• Coordinate information with interdisciplinary team for MDS assessments.
• Ensure compliance with federal and state regulations and facility policies.
• Schedule and optimize ARD section for placement.
• Identify significant changes and update care plans accordingly.
• Develop comprehensive care plans from MDS data with the interdisciplinary team.
• Lead care plan meetings with residents and families.
• Maintain documentation and ensure timely submissions of MDS to databases.
• Provide MDS training and serve as a resource for staff.
• Participate in audits, surveys, and quality improvement initiatives.
• Support reimbursement optimization with billing and finance departments.
