Job summary

East Glen position with Noland Health Services seeks a professional to oversee MDS, Raps, Triggers, and care plans, ensuring data aligns with resident needs and regulatory requirements. You will develop care plans, coordinate QA tracking, and maintain calendars while guiding staff and physicians through the MDS process.

The role emphasizes timely submission to state agencies, ongoing education for teams, and ensuring accuracy in MDS data and assessments to support quality resident care.

Qualifications

• Degree in nursing, social work or dietetics required.
• Minimum of two years of clinical experience, preferably in a Long Term Care setting.
• Must be knowledgeable of the MDS process.

Responsibilities

• Develop and complete the MDS, Raps, Triggers, and care plans timely and in compliance with regulations.
• Maintain policies and procedures for data gathering related to MDS and care planning.
• Coordinate and maintain a tracking system for Quality Assurance of MDS, Raps, Triggers and care plans.
• Provide orientation and in-service to staff, family and residents, and physicians as necessary.
• Ensure submission of all MDS/care plan information to the State timely and accurately.
• Participate in care plan meetings and notify all team members, family, and resident.
