Job summary

Avir Trilogy in El Paso, Texas is seeking an MDS Coordinator to complete the RAI process for residents in accordance with state and federal regulations.

You will manage care plans, ensure compliance with LTAC standards, supervise nursing staff, and report to the Director of Nursing. This role requires RN or LPN licensure in Texas and at least one year of supervisory experience in a hospital or skilled nursing setting.

Qualifications

• Must have licensed RN or LPN in Texas.
• At least 1 year of supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Working knowledge of Long-Term Care operational standards per Federal Register, Conditions of Participation and State regulations.
• Ability to read and write in English.

Responsibilities

• Complete the RAI process for each resident.
• Develop and implement the care plan in collaboration with the care team.
• Provide guidance to staff and ensure care plans are used in daily care.
• Report to the Director of Nursing Administration on issues and QA data.
• Review charts to ensure nursing care aligns with the care plan and resident response.
• Participate in committee work (Care Plan, QA, Utilization review).
• Maintain compliance with Texas policies and procedures.
