Job summary

Extended Care is seeking a dedicated MDS/Care Plan Coordinator in Illinois to ensure accurate resident assessments and care plans. You will communicate conditions to families, coordinate care plans, and ensure regulatory compliance with JCAHO, OBRA, IDPH, and HFS standards.

You will oversee MDS assessments, coordinate care plan conferences, and lead staff training on documentation and EMR use while collaborating with nursing and therapy teams.

Qualifications

• Current State of Illinois LPN/RN license.
• AANAC certification preferred.
• MDS experience required.
• Strong organizational, planning, and managerial skills.
• Working knowledge of nursing services, nursing administration, rehabilitation, and EMR systems.
• Ability to initiate, complete, and update care plans efficiently.
• Experience conducting staff training on care plans and EMR use.
• Knowledge of JCAHO, OBRA, IDPH, and HFS documentation standards.

Responsibilities

• Oversee completion of MDS assessments upon admission, readmission, quarterly, annually, and changes.
• Develop and maintain monthly MDS and Care Plan conference schedule.
• Ensure timely completion and accuracy of MDS sections across departments.
• Collaborate to resolve MDS-related issues and provide re-education.
• Review diagnosis coding with physicians and update ICD-9/ICD-10 coding.
• Lead weekly MDS pre-planning and Medicare meetings for compliance.
• Educate staff in EMR charting and care plan goals.
• Communicate care plans and progress to residents, families, and staff.
• Conduct quarterly in-service training for nursing staff.
